ProviDRs Care Network is proposing services beyond telephone calls and written materials.  This program delivers extensive hands-on care management services that rely on evidenced-based practices, including:

Direct Patient Care

Advocacy:  finding a provider for an insured, maximizing health care benefits, minimizing health care expenses.

Assessment:  urging better use of benefits, promoting chronic disease management, conducting home safety inspection, understanding health issues and medications.

Education:  promoting healthy lifestyle, teaching health risk management, understanding health care costs.

Coordination:  arranging for specialty consults, utilizing community resources, working with insurance case managers and health care providers.

Monitoring:  implementing prevention and health assessment activities, overseeing health status.

Evaluation:  conducting routine follow-up to discuss concerns or new issues, observing progress toward established goals.

Hospital Discharge Planning:  working with the patient, network, hospital and group to best utilize best resources for continued patient care after discharge.

When individuals who are sick or injured receive direct support and practical guidance in managing their medical needs, improved health outcomes are possible.  We believe these improved outcomes can then translate into health care savings for employers.

The anticipated outcomes of the Care Management program are simple and direct:

- Assist employees and/or their dependents in managing their health care conditions using evidence-based care management methods.
- Assist employers in managing their health care costs by implementing direct care management procedures and services.

We anticipate the support offered by skilled care managers will enable both employees and employers to realize improved health care outcomes and efficiencies.
