When individuals who are sick or injured receive direct support and practical guidance
in managing their medical needs, improved health outcomes are possible. We
believe these improved outcomes can then translate into health care savings for
employers.
The anticipated outcomes of the Care Management program are simple and direct:
|
- |
Assist employees and/or their dependents in managing their health
care conditions using evidence-based care management methods. |
|
|
|
|
- |
Assist employers in managing their health care costs by implementing
direct care management procedures and services. |
We anticipate the support offered by skilled care managers will enable both employees
and employers to realize improved health care outcomes and efficiencies.
